To register at our practice you will need to complete a registration form which can be collected from the main reception desk or downloaded from this website. Please ensure that as much information is completed as possible for ease of registration. We will update part of your records from your completed form and questionnaire and it helps us greatly if your information is accurate and up to date, for example blood pressure, smear tests, mammograms etc.
New Patient Registrations
Are you in our catchment area? Generally we cover the W4 area although we extend to The Vale, Acton through to Brentford, Gunnersbury Lane, Boston Manor and Syon Park.
When you have filled in your registration form we would like you to provide the following documentation:
- Photo ID – Passport/Driving Licence (If you do not have a photo ID please bring in your original Birth Certificate)
- Full details of your previous GP
- A Utility bill, gas, electric, telephone, no more than 3 months old or a tenancy agreement to confirm your address.
- A completed GMS1 form. Please print, complete and bring into the practice to register.
PLEASE NOTE – if you are registering a child who has been adopted, we will need to see all adoption documentation.
Once the above have been completed and checked you will be registered on our practice list. The Local Health Authority will be notified of your change of Doctor and your clinical records will be requested. The arrival of notes can sometimes take a few weeks.
