West4GPs has a facility allowing patients to use the internet to book appointments with a GP, request repeat prescriptions for any medications you take regularly and look at your medical record online. To register for this service you must complete the form below and bring in ID to show the receptionist. Once your access has been approved by a GP, your login details will be emailed to you.

Prescription Request

If you think that you may need a prescription medication, please make an appointment with the Doctor. If you have previously been given a prescription for a straightforward condition, such as hay fever, the doctor may be able to issue this without having to see you. Please ask the Receptionist. The local Pharmacies are able to give advice on non-prescription medication.

Repeat Prescription

We have a computerised repeat prescription service. Repeat prescriptions are normally for patients with long term conditions who receive regular treatment. The Doctor will decide whether you are able to have a repeat prescription. From time to time we will ask you to make an appointment for a medication review with either the Doctor or the Practice nurse. We may not be able to continue to issue prescriptions if you are overdue for a review. This is for your safety. We do this to monitor your illness and medication. You are able to order a repeat prescription by coming into the surgery when the reception is open, by post or online here.

We cannot take repeat prescription requests over the telephone or by email unless you are a housebound patient. Wherever possible please give the full name of any medications that you are requesting to avoid confusion.

Please allow two working days for repeat prescription requests to be processed.

You are unable to use this facility to request medication that does not appear on your repeat list on screen. For items not included please make an appointment to see a clinician.

Travel Consultations

West4GPS offer travel consultations.  We respectfully request that patients organise their vaccinations around 6-8 weeks prior to travel.  Patients will need to book a 30 minute consultation with our Practice Nurse, completing a travel questionnaire prior to the appointment.  It would also be useful to bring in copies of any previous vaccinations records that you have.

We cannot provide emergency travel appointments, but where possible will endeavour to offer the most convenient appointment.

Non NHS Work

The Government’s contract with GPs covers medical services to NHS patients but not non-NHS work. It is important to understand that many GPs are not employed by the NHS; they are self-employed and they have to cover their costs – staff, buildings, heating, lighting, etc. – in the same way as any small business.

In recent years, however, more and more organisations have been involving doctors in a whole range of non-medical work. Sometimes the only reason that GPs are asked is because they are in a position of trust in the community, or because an insurance company or employer wants to ensure that information provided to them is true and accurate. Did you know using GP at Hand de-registers you from our Practice?

GP at Hand is a smartphone app which allows you to book video GP appointments anytime anywhere 24/7, and you can be offered a face-to-face appointment if required. This all seems brilliant but it’s not. In fact GP at Hand is destabilising general practice and failing to provide equal access.

Each time a patient registers with GP at Hand they become de-registered from their current practice, and the funding goes with them. By delivering care disproportionately to the worried well, GP at Hand undermines healthcare delivery to those who need it most.

Should you need to use GP at Hand please note you will be deregistered from the practice and you will need to re-register to access our services. If you have used GP at Hand then please complete a new registration form.

Chiswick Health Centre have an active Friends Group who meet regularly with residents of the borough to bring together like minded people who are concerned about the future of General Practice and the impact change will have on the future of the Health Centre and its Users.

Referral Centres

As part of Hounslow CCG we have the option of a wide range of specialist centres. The policy is that we use specialist centres where we believe the patient is being offered the best service and quality of care, as guided by our local GP Commissioning Group.

The hospitals we most commonly use include: Charing Cross Hospital, Hammersmith Hospital, Queen Charlottes Hospital, West Middlesex Hospital and Chelsea and Westminster Hospital.

Medical Student Training

West4GPs is a student training practice, assisting medical students in their preparation to become a doctor. At present, medical students from Imperial College, King’s College and occasionally other medical schools are taught in some surgeries. We also undertake training of other healthcare professionals.
