Request a doctor’s appointment/advice

Please note all doctor’s appointments are now done over the telephone in the first instance and the doctor will give you a face to face appointment if clinically necessary. Appointments  are book on the day only.

You can request a doctor call or advice by:

  • Calling reception at 8am or 12pm on 0208 630 3518
  • During the week Monday to Friday for non urgent requests you can complete a Patchs consult form from the following link: https://patchs.ai/practice/West4GPs
  • If you do not have access to the internet, please call reception on 0208 630 3518
  • For any emergencies or home visits, please call reception.

Order a repeat prescription

Please note the online service is only used for items, quantities and doses that appear on your repeat list, and is not for repeat dispensing. If you would like to request something that does not appear on your repeat list, you will need to complete a Patchs form.

Request a medication review

  • Login to your Systmonline account and go to Request Medication
  • Then click Custom Request and type in the text box that you require a medication review and for what medication
  • If you do not have online, then you will need to bring your repeat slip or a written note to the surgery

Request contraceptive or HRT medication

  • You MUST have an up to date blood pressure reading with your request.
  • If you have a reading from home or the pharmacy, then please complete a Patchs form (https://patchs.ai/practice/West4GPs) or bring a note to the surgery with your reading on.
  • If you do not have a reading, please come and use our POD machine at reception and complete our paper request form.

Book a blood test, smear, nurse appointment etc

  • Please call reception on 0208 630 3518.
  • We cannot book appointments via email or Patchs, and the online booking service is disabled
